We will help walk you through the steps to request your medical records
Requesting Your Medical Records
Victory Sports Medicine & Orthopedics is committed to protecting the privacy of your medical information. To safeguard your privacy, you must complete a medical records release form before records can be sent. On the form, you will need to indicate what records are to be released and where they are to be sent.
Step 1: Complete form
Please download and complete this form to request that Victory Sports Medicine & Orthopedics release your medical records to another provider. Please note that failure to complete all required elements on the form may result in a delay in processing your request.
Tip: The medical records release form will ask what specific information you would like copied. A medical record can often times be hundreds of pages long, so being selective is important. This will help us ensure that you receive the records you need – and not the ones you don’t. The set of records one might request for personal use, for example, can be different from the set of records sent to a doctor for continuing care. If you are uncertain, contact our office and tell us what you want your records for and then we can tell you what documents you will need.
Step 2: Send completed form to us

Frequently Asked Questions
Medical Records Requests
How long will it take to receive copies of my medical records?
A: Fulfilling record requests can take time, so plan ahead and don’t expect to receive your records the day you request them. Once received, each medical record request requires personal attention. Depending upon the amount of records, some requests can take up to 10 to 14 days. However, our dedicated team of professionals makes every effort to process them sooner.
Is there a fee for copies of my medical records?
A: Yes, as per our financial policy, patients requesting copies of medical records will be charged $0.75 per page to offset some of our costs of performing this service. There is also a $5 charge for CDs of X-Ray or MRI images. These fees follow New York State guidelines and are subject to change.
Can I have my medical records faxed to me?
A: No, due to the confidentiality of the records we are not able to fax them to you. We can mail them to you or you can pick them up in person at our office.
